Covid hair loss — the London trichologist's guide, four years on

Post-Covid hair loss is telogen effluvium triggered by the febrile, inflammatory phase of infection. Four years of London trichology data now show a consistent pattern — and consistently good outcomes.

Why Covid triggers it

Fever above 39°C, systemic inflammation, and — for some — the metabolic stress of long Covid all push follicles into telogen synchronously. This isn't unique to Covid; any viral illness with fever can do it. Covid was just widespread enough to make it visible.

Treatment protocol

  • Ferritin, TSH, vitamin D, B12 — correct any deficiency
  • Minoxidil 5% topical — optional, accelerates regrowth
  • Nutritional adequacy (protein 1.2g/kg/day for the recovery period)
  • Reduce concurrent stressors where possible

When to worry

If shedding continues beyond 6 months, if hair regrows visibly thinner, or if patchy loss appears — see a trichologist. These can indicate chronic TE, unmasked female-pattern loss, or (rarely) an autoimmune trigger.

Does the vaccine cause hair loss?+

Reported rarely, mechanism unclear. Post-infection loss is much more common.

Can long Covid keep hair loss going?+

Yes — persistent inflammation can drive chronic TE. Investigate underlying long-Covid management.

Will it come back with reinfection?+

It can, though usually milder. The follicles are the same; the trigger is smaller the second time.
