The historic reason women were rarely transplanted was assumed inadequate donor. That assumption is often wrong. Modern trichoscopy shows that many women with FPHL retain excellent occipital donor — enough to make transplant a genuine option.

- Traction alopecia in stable phase
- Hairline lowering (aesthetic, non-loss)
- Post-surgical scars (facelift, brow lift)

Who doesn't (yet)
Diffuse Ludwig 3 without stable donor. Active telogen effluvium. Unmedicated FPHL — because progression will make the transplanted area look surgically stranded. Medical therapy for 12–18 months first, then reassess.
The London female protocol
Longer consultation (90 min minimum), full hormonal panel, ferritin, thyroid. Trichoscopy of donor before yes/no. Usually paired with minoxidil ± spironolactone long-term.
The right answer for many women is medical for two years, then a small transplant to restore density.
